birth, which is the city or town and state if you live in the US. If you do
not live in the US give the month, day, year, time AM/PM of your and
the closest largest town and country. Please make sure spelling is accurate.
Birth information should be taken from
the birth certificate for accuracy and not from baby books or mom’s memory.
An error of four minutes can make a difference.
If you do not want to supply your birth data
and not not want to use your own birth chart in class preview, this is acceptable.
But you still must submit this form without your birth data by clicking on
the submit button below and then proceed to the registration form.
When you have completed filling in your birth data click on the submit
button and it will send your information via email to me. You will receive
an email back with the link and password.
